TEST FORM

Please submit test forms with appropriate fees prior to testing. Cash only please.

* Tests are $45
* ASU Membership Dues are $45 and only need to be paid once a year
* Belts are $5

GI ORDER FORM
There is usually a "Gi run" once a semester. Please submit completed forms and payment to Jeff Scanlan or Jim Carr. Cash only please.
